If a document asks for the "Name and Address of Witness (Type or Print)" what would you do?

The document is a standard form (everyone who is sent the document receives the same form). No signature of the witness is required and no notary signature is required.

Doesn't make much sense.

Sorry...let me be clearer.

Prior to my father's death he signed a document which asked only for the Name and Address of Witness (Type or Print). No signature of the witness was required and no notary signature was required. He gave them what they asked for, the name of a witness and their address.

Now, its being said the witness did not sign the document even though that isn't what the document asked for.

Is there a lawyer in the house that is not a friend of C Jones?

If a doc is in error it is in error. I believe it is up to the person(s) wanting a proper doc to see to it that it is a proper doc with proper signatures. In other words, someone has to be responsible that a contract is executed properly in the first place.
